This clinical trial examined the safety and efficacy of volumetric modulated arc therapy (VMAT) and RapidArc (RA) as intraoperative tumor-bed boost in the treatment of head and neck cancer.
Methods {#Sec2}
=======
From May 2012 to April 2013, 29 patients with advanced head and neck cancer were enrolled. The disease stage was T3 or T4 for 14 patients and T1-2 for 15 patients. The primary tumors were located at supraglottic larynx (n=8), hypopharynx (n=8), oropharynx (n=12), and oral cavity (n=3). VMAT was used for 23 patients and RA for 6 patients. The median tumor dose was 60Gy in 20 fractions (range 50.5-63Gy) for VMAT and 70Gy in 25 fractions (range 63-71Gy) for RA, with a daily dose per fraction ranging from 3Gy to 4Gy. The margin dose was set within ± 2mm.
Results {#Sec3}
=======
The median follow-up period was 24 months (range 6-45months). The 3-year local control rate and the disease-free survival rate were 86% and 59%, respectively, with no significant difference between VMAT and RA. However, there were more acute complications (skin or mucosa) in RA group. No late complications were observed.
